Part 1: Understand the various roles of the interprofessional care/service team members.

Part 2: Maintain responsible, respectful work relationships with the care team.

Part 3: Provide ongoing, relevant client information to team members.

Part 4: Assist, work with others and request guidance when needed.

Part 5: Accept, act upon and request constructive feedback from the care team.

Part 6: Participate in the development, efficacy and modifications to the client’s care plan.
